Alexandra Hospital is the first integrated hospital, where acute care and rehabilitative care is provided by the same care team, under one room. This means that you can focus on getting better, as the single care team takes you through recovering from illness, all the way to recovery and rehabilitation.
This will make your stay at Alexandra Hospital a hassle-free, and a potentially shorter one, without the need to switch to other community hospitals.
Our outpatient care model is new and unique – where one doctor and one dedicated care team serve you. Moreover, our model of care is administered by our core care programmes – Be Better (preventive care), Get Better (acute care), Live Better (chronic illness management) and Age Better (healthy ageing).
With one principal doctor, supported by a care team, multiple medical conditions can be taken care of, without the need to see multiple doctors.
After being discharged from inpatient care, or after your outpatient appointments, you will continue to be served by our out-of-hospital-care programme. We will bring together community members, form primary care to daycare and social services, to help fulfil your health and wellbeing needs.
This is so that you can live better, in the comfort of your own home and community.
